Alexander disease is a set of genetic disorders called leukodystrophies that the development affect the myelin sheath, the fatty coating which acts as an insulator-on nerve fibers in the brains. Alexander disease is considered an autosomal dominant disorder, which copy of the altered gene in each cell means is sufficient to cause the disease. Almost all cases result from new mutations in the gene and occur in people with no history of the disorder in their family. Children with hydrocephalus caused by Alexander's disease usually have an increased intracranial pressure and a faster progression of the disease. In general, the earlier the age at onset of Alexander disease, the more severe and rapid course. So unaffected parents of a child with ALX are a low risk for another affected child.
Alexander disease, which affects mostly men, usually begins about 6 months old. Signs and symptoms of the infantile form usually include an enlarged head and brains, seizures, stiffness in the arms and legs, mental retardation, and delayed physical development. Less frequently, will start later in childhood (juvenile form) or in adulthood. Common problems in young and adult forms of Alexander disease, speech disorders, swallowing problems and poor coordination. Infantile Alexander disease leads to complaints In the first two years of their lives, while some children die in the first year of life, a greater number of live are 5-10 years old. Alexander disease belongs to leukodystrophies, a group of diseases that the growth or development of the myelin sheath influence. In Alexander disease, the growth of the white matter of brains, or myelin sheath is affected.
Cerebellar disease also occurs in this condition. In most cases, Alexander disease seems to occur randomly for unknown reasons, no family history of disease. In a very small number of cases is thought that the disorder may have more than one family member affected. Affected children survive a few weeks to several years. The juvenile form presents usually between four and ten years, sometimes in the mid teens. Survival is variable ranging from early teens to the 20s-30s. Prior to the identification the genetic characteristics, other names were used for the clinical features and pathology of Alexander disease, such as hyaline panneuropathy and describe dysmyelinogenic leukodystrophy. These terms are not used anymore.
Because the genetic defect in Alexander's disease is known, genetic testing on a blood sample is used for most cases of Alexander disease to diagnose. There is no medication for Alexander disease. Treatment, which is symptomatic and supportive, mainly focus on the general care and feeding require antibiotic therapy for infections and management of its complications, such as anticonvulsant therapy for epileptic seizures. A diagnosis can also be suggestive made of the clinical symptoms, including enlarged head size, combined with negative tests and radiological studies for other leukodystrophies. Surgery, including the placement of a feeding tube and / or shunting for hydrocephalus, may also be required. Orthopedic surgery for scoliosis has been reported in a case of Alexander disease.
